POLYMORPH HYPERSEGMENTATION IN THE ELDERLY

In 47 elderly patients admitted to hospital, and having anaemia, macrocytic red blood cells or hypersegmentation of the polymorphs, a polymorph lobe count was performed, and the results compared with other evidence of megaloblastosis and haematinic deficiency. Routine inspection of the blood film for hypersegmentation correlated well with the lobe count, as did the serum vitamin Bn and to a lesser extent, the white blood cell ascorbic acid levels. It was concluded that the hypersegmentation of neutrophil polymorphs in the blood film is a valuable guide to the presence of haematinic deficiency likely to cause megaloblastosis in the elderly.
